(with quotes as above) in Google scholar

Followed the links from the first URL until

http://arno.unimaas.nl/show.cgi?did=10724

then "file 2", which is:

http://arno.unimaas.nl/show.cgi?fid=7134

and was able to download a .pdf

To my surprise a scanned (not very normal
for a 2002 document) 16.7 Mb with the
complete 281 pages.

I did not find the time to read it yet, but
it is in my "to do" list.
